Eli Manning Sidelined Due to Plantar Fasciitis

In the 4th quarter against the Kansas City Chiefs, Eli Manning dropped back for a pass, planted his foot and dropped to the ground after completing the pass. He immediately grabbed his heel. Manning continued for one more play and then walked off the field. An MRI later reviewed inflammation of his plantar fascia. The plantar fascia is a long connective tissue structure in the bottom of the foot. The tissue is similar in strength and composition to a ligament. The plantar fascia starts at the heel bone and travels to the ball of the foot, with web-shaped bands extending to the inside arch and the outside area of the foot. The plantar fascia functions to support the arch. Manning was diagnosed with plantar fasciitis (plan * TAR fash * ee * I * tis), tearing and inflammation of the plantar fascia. This condition is typically an overuse injury which develops due to repetitive stress of the plantar fascia. Small microtears cause inflammation and weakening of the ligament-type structure which can result in further tearing, inflammation and pain.

Over two million individuals suffer with plantar fasciitis each year. The condition ranges in severity and can last for 2 weeks or can linger for more than a year, disabling some individuals for even longer periods. The condition can be incredibly painful and can significantly decrease the ability to generate power during the push-off phase of gait. For a quarterback, this means a loss of power during the initiation of the pass and pain when planting the foot.

It was reported that Manning had previously complained of heel and arch pain. He had most likely been suffering with mild plantar fasciitis. Once the ligament has torn and inflamed, even if only to a small degree, it loses it's strength and becomes more susceptible to injury. When Manning dropped back for the pass, he over-stretched his back leg. When he planted his back foot, he placed it parallel to his leg with his weight on the ball of his foot. The back foot should be placed more perpendicular to the leg with more even weight distribution over the entire foot. When weight is placed on the ball of the foot and the leg is outstretched, this placed excessive stretch on the plantar fascia. In most conditions, the fascia should be able to support this amount of force, but if the fascia is weakened due to prior injury, it becomes susceptible to further tearing.

For most individuals, plantar fasciitis develops as a result of repetitive stress and there is not one incident or traumatic event that causes the injury. The pain is generally sharp at the bottom of the heel at the first step in the morning and tends to work itself out after walking. The pain may be deep and dull, or sharp and stabbing throughout the remainder of the day. Activities such as going up and down stairs and hills, carrying objects or heavy items or squatting will aggravate it.

The initial treatments for plantar fasciitis include rest and avoiding aggravating activities as mentioned above. During the first two weeks twice daily icing for 15 minutes and alternating between a hot pack and a cold pack for 5 minutes each for a total of 30 minutes will decrease the inflammation. Wearing supportive shoes and avoiding going barefoot will significantly decrease the stress in the arch. Stretching the calf and the plantar fascia will accelerate healing, this is especially helpful before stepping down first thing in the morning. If the condition progresses, night splints, orthotics, steroid injections and physical therapy techniques such as ultrasound and iontophoresis may be initiated. Manning has started with conservative treatments and will have his foot taped to reduce stress through the arch and support the plantar fascia. He is not scheduled to start in Sunday's game.
